Tiếp tục lấy bối cảnh một hòn đảo đầy những Zombie khát máu, người chơi sẽ vào vai một trong số những người thuộc một nhóm, cùng dựa vào nhau để chiến đấu và tồn tại. Trên hòn đảo nhiệt đới đó, bạn nhận nhiệm vụ, đi thực hiện, lên cấp, nâng cấp vũ khí, và tiêu diệt Zombie. Bạn có thể vừa sử dụng các loại súng khác nhau, vừa có thể chiến đấu bằng rất nhiều các loại vũ khí thô sơ, và thực hiện nhiều chiêu cận chiến khác nhau. Đặc biệt, yếu tố môi trường đóng vai trò khá quan trọng trong gameplay của trò chơi.
